PRFM Membership
(Direct Primary Care)
Many of you understand the relentlessness of healthcare bills. Unfortunately, many of us cannot afford expensive insurance plans. Others may have a plan, but it requires not just the purchase of the plan itself, but an often high deductible as well as office visit co-pays.
Simply put, insurance company's are in the business of making money, not saving you yours. This makes seeking health care a financial burden to many individuals and families.
Few of you may be aware that primary care is the least reimbursed health service in our country, though, if we may be so bold, it is certainly one of the most important. The sad truth is insurance has become a barrier to seeking healthcare.
We at PRFM have decided to do something different - we are thrilled to announce we have a service to save you money, improve your access to care, while also supporting our small business model. It's called the PRFM Membership, known also as a DPC program, or retainer practice.
You can think of this model as a gym membership, but with your primary care clinic. Members have unlimited visits for one monthly fee, no co-pays, no deductibles.
Meanwhile, PRFM is empowered to serve patients according to our mission while having the financial control and stability needed to maintain a sustainable business model.

Membership Packages:
Individual Adult: $70/Month
Adult Couple: $130/Month
Single Parent Family (One adult + Unlimited Children (2-24 years): $115/Month
Two-Parent Family (Two adults + Unlimited Children (2-24 years): $170/Month
The membership fee is due immediately when enrolling in the DPC membership program and the first of the month thereafter or annually depending on individual preference. The contract will automatically renew each year unless PRFM is given 30 days’ notice to terminate the agreement. If an individual is delinquent on payment they will have 90 days to bring accounting up to date. After that, the account will be sent to collections. If a client refuses to establish a payment plan they will receive 30 days’ notice that they will be discharged from the practice.
Reasons for being discharged from the practice:
- Patients who do not keep their account balance up to date
- Verbal or physical abuse directed to providers or staff
- Non-compliance with care i.e. missing appointments, recommended testing, and follow-up.
- Patients who are “provider” shopping to receive medications such as opioids or benzodiazepines.

DISCLOSURES:
Phoenix Rising Family Medicine (PRFM) is not insurance.
PRFM provides only the limited scope of primary care services specified in the retainer medical agreement listed in Appendix A.
Patients can cancel this agreement with written notice sent to PRFM. PRFM will then reimburse the patient for any unused portion of fees.
